The title as designated and fixed by the Board is as follows:

A change to the Colorado Revised Statutes mandating the disclosure of federal income tax returns for candidates for certain elected offices, and, in connection therewith, requiring that candidates for federal or statewide office file with the secretary of state copies of their income tax returns for at least the five most recent taxable years the returns were filed; requiring candidates to consent to public disclosure of the returns; directing the secretary of state to redact certain personally identifiable information on the returns and then post the returns on the secretary's website; and disqualifying any candidate who fails to timely comply with the disclosure and consent requirements.

The ballot title and submission clause as designated and fixed by the Board is as follows:
