Select the appropriate document in the entity history that first listed the information that needs to be corrected.

Entity ID Number, Trademark ID Number, Name, Trademark, and Document Number

These fields are all pre-filled with the entity information in the records of the Colorado Secretary of State. This information cannot be corrected by filing this document.

Correction of trademark information

If trademark information provided was incorrect, make a selection and enter the correct information. Only one of the following may be selected.

  • The statement of standard character trademark
    • Select “Yes” if the characters constituting the standard character trademark are incorrect. In the textbox below, enter a corrected statement of the Standard Character Trademark. A Standard Character trademark is a trademark that consists of words only, using English letters, Roman or Arabic numerals, and punctuation marks.
  • Trademark incorrectly identified as special form trademark
    • Select “Yes” if the trademark was incorrectly identified as a Special Form Trademark and should have been identified as a Standard Character Trademark. In the textbox below, enter the word or phrase, using English letters, Roman or Arabic numerals, and punctuation marks that make up the trademark in the space provided.
  • The description of the special form trademark
    • Select “Yes” if the description of the drawing of the special form trademark is incorrect. In the textbox below, enter a corrected description of the Special Form Trademark. A Special Form Trademark is a trademark that consists of words with special characters, font, formatting and/or a picture.
  • Trademark incorrectly identified as standard character trademark
    • Select “Yes” if the trademark was incorrectly identified as a Standard Character Trademark and should have been identified as a Special Form Trademark. In the textbox below, enter a detailed description of the special form trademark. Attachment of a drawing or picture of the trademark may be required if the document identified above did not include a drawing or picture of the trademark. If an attachment is required, select “Yes” under the Attach Additional Information section. After clicking on “Submit” at the bottom of the online form, the system will proceed to the Manage Attachments page. Individual causing delivery [2]

Each individual causing the document to be filed is responsible for complying with the applicable statutes.
