Correction of the form of registrant

Select “Yes” if the form of registrant as stated is incorrect.

Select the correct form of entity from the drop-down list provided. If “Other” is selected, enter the form of the entity in the space provided. 

Correction of jurisdiction

Select “Yes” if the jurisdiction under the law of which the entity is formed is incorrect or was not provided.

Select the correct jurisdiction where the registrant was formed from the drop-down list provided. The drop-down list includes all U.S. states, U.S. territories, and Canadian provinces. If the jurisdiction does not appear in the drop-down list, select “Other” and enter the jurisdiction in the space provided. 

Historical purposes

Select “Yes” if the document is being delivered for filing for historical purposes and should not correct information currently of record. 

Additional information

Select “Yes” if additional information is permitted or required by law to be included in the document. After clicking on “Submit” at the bottom of the online form, the system will proceed to the Manage Attachments page.
