If you have trouble using the form:

• Send an email to darleen.herrera@coloradosos.gov. Include:
  
  
  • Organization's name,
  • Address where the class will be held,
  • The number of people the location can hold,
  • Information about any available audio/visual equipment or amenities that would be useful for holding a class,
  • Whether you want to schedule the class for a weekday evening or a weekend,
  • Any specific times, dates, or weeks that would be convenient for a class,
  • Your name, and
  • Your phone number and email address.
